Samarium - Bone - Cancer - Treatment

Samarium-153 – short description

Samarium-153 is a radioactive isotope that is attracted to the fastest growing cells in bone.
Cancer cells are always the fastest growing cells, therefore, the Samarium goes directly to the tumor in the bone.
The Samarium is injected by intravenous which is painless and fairly quick.
Samarium has a half-life of 48 hours --- during that time isolation of the dog and supervision by qualified stuff is necessary.

After 48 hours a bone scan is done on the dog – from head to toes.
The scan will show the Samarium in any part of tumor.
The benefit of the scan is learning where other cancerous areas are, if there are any.
After that,the dog is released to go home.

During the next 6 weeks, the Samarium remains in the bone and continues to kill the cancer cells with gamma rays.
It does kill the tumor.
The danger period is 6 weeks after the injections of Samarium because the white blood cells drop suddenly, a result of
being irradiated. The dog should be kept at home and monitored for any infection.
They are vulnerable to bacterial or viral infection with their low immune response.
It’s temporary and within a short time, the white cells go back to normal.

Once the Samarium has killed the tumor, the bone calcifies and can hold the dogs weight. Samarium has been
used to treat bone cancer in woman with breast cancer and in men with prostate cancer, when the cancer goes off
into the bone.

In humans, Samarium is used not as a cure but as a way to stop the pain. I suppose since the bone cancer in these
people is not the primary cancer, it is possible to stop the progress of the disease but in dogs, the bone cancer is the
primary or first cancer and therefore, whentreated early enough,it’s very effective. The dog may need to be treated again
one year after the first Samarium-treatment.

Radioactive Isotope Treatment Samarium was developed in Norway.

Samarium-153 – kurze Erläuterung

Samarium-153 ist ein radioaktives Isotop, das auf die am schnellsten wachsenden Zellen der Knochen einwirkt.
Krebszellen sind immer die am schnellsten wachsenden / sich teilenden Zellen und dadurch wird das Samarium direkt
in den Tumor im Knochen transportiert.
Samarium wird intravenös injiziert, was schmerzlos und rasch wirksam ist.
Samarium hat eine Halbwertzeit von 48 Stunden --- während dieser Zeit muß der Hund isoliert und durch qualifizierte Kräfte betreut werden.

Nach 48 Stunden wird einen Ganzkörper - Knochen-Scan durchgeführt.
Der Scan zeigt das Vorhandensein des Samarium in jedem Teil des Tumors.
Der Vorteil des Scan-Screening liegt darin, daß eventuell andere tumoröse Zonen erkannt werden.
Danach kann der Hund nach Hause entlassen werden.

Während der nächsten 6 Wochen verbleibt das Samarium in den Knochen und zerstört die Tumorzellen mit Gammastrahlen und damit den Tumor.

Die gefährliche Periode beginnt 6 Wochen nach der Samarium-Injektion, da die weißen Blutkörperchen plötzlich absinken/zurückgehen als Folge der radioaktiven Isotopenbehandlung. Der Hund sollte daher in seiner gewohnten
Umgebung zu Hause gehalten und genauestens beobachtet werden wegen Infektionsrisiko. Das Tier ist in dieser Zeit
anfällig bezüglich bakterieller oder viraler Infektionen, da das Immunsystem herabgesetzt ist.
Dies ist ein vorübergehender Zustand und binnen kurzer Zeit erholen sich die weißen Blutkörperchen und die Anzahl
steigt an auf das normalerweise übliche Maß.

Wenn das Samarium den Tumor zerstört hat, findet ein Kalzifizierungsprozeß in den Knochen statt. Samarium wurde eingesetzt zur Bekämpfung von Knochenkrebs-Metastasen bei Frauen mit Brustkrebs (als Ursprungstumor) sowie bei Männer mit Prostatakrebs, wenn sich der Krebs auf die Knochen ausbreitete.

Beim Menschen wird Samarium überwiegend zur Schmerzbekämpfung bei Krebs eingesetzt, da der Primärtumor oft ein anderer ist und es war auch in der Humanmedizin möglich, das Fortschreiten des Tumors aufzuhalten.

Da aber beim Hund der Knochenkrebs der Primärtumor ist, ist die Samarium-Behandlung – rechtzeitig genug angewandt – sehr effektiv. Das Tier muß eventuell (je nach Schweregrad) nach einem Jahr erneut einer Samarium-Behandlung unterzogen werden.

Die radioaktive Isotopen-Behandlung mit Samarium wurde von norwegischen Wissenschaftler (Univ. Oslo) entwickelt.

Daten über behandelte Hunde, Überlebensrate und –dauer nach der Behandlung sollten in den USA (bei Barbara Bouyet) nachgefragt werden.

Prednisolon (and other corticoides) are given as well - Cortison is known to stop pain in joints as well.

- My very own experience with this - treatment of my dog (follows of accidents): Cortison-application i.m. or intraarticular works very well on this purpose, oral applied it didn't work at all -
I suppose that the reaction of an animal to treatment is as individual as the reaction of humans to treatment.

EYEDISEASES

- there is treatment (surgical) for entropion/ectropion/distichiasis/lens luxation - but I can't specify these form of treatment.

- Es gibt chirugische Behandlungsmöglichkeiten für Entropium/Ektropium/Distichiasis/Linsenluxation - aber wir kennen uns hier zu wenig aus, um ausführlich darüber zu berichten.

HEARTDISEASES

Medical treatment, depending on the form of disease.
Digitalis glycosides, e.g. Lanitop & ACE-inhibitors are usually applied for dilat. cardiomyopathy; diuretics could be required, depending on the stage of disease.

Medikamentöse Behandlung, abhängig v. d. Art der Erkrankung.
Digitalispräparate & ACE-Hemmer werden eingesetzt bei dilat. Kardiomyopathie; Diuretika können - je nach Stadium d. Erkrankung - erforderlich werden. Seit ca. 2 Jahren gibt es ein Kombi-Präparate (Vetmedin®), welches den Einsatz von ACE-Hemmern erübrigt und gut vertragen wird.

Hernia's, inguinal, umbilical, causes and recommendations

Usually surgical treatment (it's a simple operation, the puppy can leave the hospital at the same day), depending on the size and type of hernia.

Everything known to support connective tissue, would be of benefit (e.g. silicea vitamin E, biotin, other minerals and maybe enzymes)., esp. if given to the bitch in the last half of pregnancy and during lactation.

HYPOTHYREOIDISM

- Thyreostaticas*, just like for humans (e.g. L-Thyroxin®) are usually the treatment of choice, dosage depending on the degree of hypofunction and size/weight of dog.
